Twin Oaks Post Acute Rehab provides a continuum of care that includes professional nursing care, rehabilitative therapy and support services to residents enabling them to maintain their independence.

We are a 144 bed Skilled Nursing Facility designed to meet both short- and long-term care needs. 24 newly decorated private rooms are available for short-term post acute care. We participate in Medicare/Medicaid programs and contract with several HMOs.
