Frontier Rehabilitation and Extended Care Center's continuum of care includes professional nursing care, rehabilitative therapy and support services for residents enabling them to maintain their independence in a home-like setting.

We are a 140 bed Skilled Nursing Facility designed to meet both short- and long-term care needs. 15 newly decorated private and 25 semi-private rooms are available for short-term care. We participate in Medicare/Medicaid programs and contract with several HMOs.
